Request Description

Can you please inform me whether you hold on record the number of social care assessments that have been undertaken in each year from 2010 to present in the following categories, a) total number of assessments, b) assessments for each recorded illness group, c) assessments for people with neurological conditions, d) assessments for people with M.E./CFS.

Please can you also provide the number of claimants that successfully received a care package in each of the above groups.

If a service was offered to patients was it a service that included a) short-term care to maximise independence, b) long-term home care, c) assistive equipment. Similarly, where people received a social care package was it for a) personal care, b) access and mobility, or both.  

If this information is not held centrally I would be very grateful if you could give an overview as to why this is the case or where the data would be held.
